RE: Seminar 2010 Speaker Information
Dear Presenter,
The California Society of Health-System Pharmacists (CSHP) is pleased that you have accepted the invitation to speak at Seminar 2010. Seminar 2010 will be held October 21-24 at the Hilton Union Square in San Francisco, CA. We are sure that your participation will add significant value to our CPE program.
Included in this section of the site is your Speaker Agreement packet. Please read through this document, making sure to check where indicated before submitting. Please be sure to print a copy for your records. Additionally, in order to meet ACPE requirements and to expedite accreditation for your program, we are requesting your program materials to be sent to us no later than August 15, 2010. An itemized list of requirements is included on the Speaker Agreement packet.
It is the speaker’s responsibility to make travel arrangements. Please contact the Hilton Union Square at 415-771-1400 to obtain the negotiated rate of $229 per night. Make sure you mention CSHP Seminar to receive the discounted rate. This rate is available until all rooms are reserved. If you are flying to San Francisco, please make your reservations at least one month in advance to ensure the lowest possible rate.
You will receive free, full-time registration to Seminar 2010; a registration form will be mailed to you to fill out and return. Please check in at the Seminar 2010 registration desk no later than one hour before your presentation.
